One of the most common fears patients have when considering the dental implants without bone grafting is that years of missing teeth have left an insufficient amount of bone and that they are told that the surgery is not possible, or that a long and expensive grafting procedure is necessary even before implants can be considered.

Bone loss occurs after tooth loss because the jawbone no longer receives the stimulation it once received from the tooth's roots. This stimulation, typically provided by the forces of chewing and biting, helps to maintain bone density. When a tooth is lost, this stimulation ceases, and the body begins to resorb the bone in that area, as it is no longer deemed necessary. This process is known as disuse atrophy. Over time, this can lead to a significant reduction in the volume and density of the jawbone where the tooth was extracted.

After a tooth is lost, the jawbone in that area no longer receives the stimulation that normally comes from chewing and biting. Without this stimulation, the body gradually begins to reabsorb the bone, a process known as bone resorption.

This process can start after just a few months and tends to continue over time. The longer a tooth is missing, the more likely some bone loss will occur, which is why patients who have been missing teeth for years are sometimes told they don't have enough bone for traditional implants.

Technique 1: Strategic Implant Placement

Bone loss does not occur uniformly along the jaw. Some areas maintain greater density and strength, and our surgeons use precisely these stronger zones to safely place implants.

With the All-on-4 technique, the posterior implants are often inserted at an angle rather than vertically. This allows for deeper and more solid bone to be reached, enabling many patients with bone loss to receive implants without the need for grafting.

Technique 2: Zygomatic Implants

In cases of severe bone loss in the upper jaw, zygomatic implants can be used. These are longer implants that are anchored to the zygomatic bone, i.e. the cheekbone, which remains dense and stable even when the jaw has lost a significant amount of bone.

This advanced technique makes treatment possible even for many patients told they are unsuitable for traditional implants. In many cases, it completely avoids the need for bone grafting.
